อาการหลง ๆ ลืม ๆ เพียงเล็กน้อย ไม่ได้สร้างปัญหาให้การดำเนินชีวิตในประจำวันติดขัด แต่ทราบหรือไม่หากปล่อยทิ้งไว้ อาจเป็น จุดเริ่มต้นของโรคอัลไซเมอร์และกลายเป็นโรคสมองเสื่อมขั้นรุนแรง ในที่สุด ส่งผลให้ผู้ป่วยมีคุณภาพชีวิตที่แย่ลงทั้งกับตัวเองและคนรอบข้าง
ดร.นพ.โยธิน ชินวลัญช์ อายุรแพทย์สมองและระบบประสาท โรงพยาบาลกรุงเทพ เปิดเผยว่า ปัจจุบันจำนวนผู้ป่วยโรคสมองเสื่อมเพิ่มมากขึ้นเรื่อย ๆ ตามอายุที่มากขึ้น จากสถิติของประเทศสหรัฐอเมริกาในปี ค.ศ. 2000 เฉพาะโรคอัลไซเมอร์มีผู้ป่วยประมาณ 45 ล้านคน เป็นเพศหญิงมากกว่าเพศชาย สำหรับในประเทศไทยก็อยู่ในช่วงที่ประชาชนมีอายุยืนยาวมากขึ้น ในเพศหญิงเฉลี่ยอายุประมาณ 82 ปี ส่วนผู้ชายประมาณ 79 ปี ฉะนั้นในวัยนี้ความเสี่ยงของโรคสมองเสื่อมจะสูงขึ้นตามอายุ ดังนั้นบ้านเราจึงเป็นประเทศหนึ่งที่มีผู้ป่วยภาวะสมองเสื่อมสูงขึ้นเรื่อย ๆ
ภาวะสมองเสื่อมมีหลายรูปแบบที่พบมากที่สุดคือโรคอัลไซเมอร์ ประมาณ 65 เปอร์เซ็นต์ของโรคสมองเสื่อม โดยภาวะของสมองเสื่อมในบุคคลที่เป็นอัลไซเมอร์นี้ไม่ได้เกิดขึ้นทันทีทันใดแต่เป็นโรคที่เป็นทีละนิด ๆ สะสมก่อน ซึ่งกระบวนการนี้จะมีพยาธิสภาพในสมองเริ่มต้นจากภาวะปกติและมีวิวัฒนาการจนกระทั่งมีอาการทางคลินิก จึงเรียกว่าเป็นอัลไซเมอร์เต็มตัว ซึ่งอาการจะเริ่มต้นด้วยการสูญเสียความจำ เช่น หลงลืมบ่อยขึ้น ได้แก่ ลืมกุญแจ ลืมปิดประตูบ้าน ลืมปิดไฟ โดยไม่มีความผิดปกติทางร่างกายอย่างอื่น เช่น การเดิน การพูด ระบบประสาทสัมผัส แต่อาการเหล่านี้จะผิดปกติขึ้นในช่วงปลาย ๆ ของตัวโรค เช่น เดินไม่ได้ ถือเป็นภาวะที่ค่อนข้างช้าเกินไปในการรักษา ปัจจุบันจึงยังไม่มีการรักษาที่หายขาด มีแต่ให้ยาช่วยบรรเทาอาการทำให้คนไข้มีคุณภาพชีวิตที่ยังไม่เสื่อมถอยเร็วจนเกินไป
นอกจากอัลไซเมอร์แล้วยังมี “โรคสมองเสื่อมชนิด Lewy Body Dementia” หรือ LBD พบได้ 30 เปอร์เซ็นต์ของผู้ป่วยที่มีโรคสมองเสื่อมและส่วนใหญ่เกิดขึ้นในเพศชาย โดยโรคนี้จะทำลายเซลล์สมองเฉพาะสมองส่วนหน้า พบในคนไข้อายุประมาณ 65 ปี หรือบางคนมากกว่า 45 ปีก็เริ่มเป็นได้ ถึงแม้จำนวนจะน้อยกว่าอัลไซเมอร์ แต่มีอาการรุนแรงกว่า เพราะจะช่วยเหลือตัวเองไม่ได้ภายใน 2 ปี “โรคสมองเสื่อมชนิด Fronto-Temporal Dementia” หรือ FTD พบประมาณ 5 เปอร์เซ็นต์ในผู้ป่วยที่เริ่มเกิดโรคสมองเสื่อมในอายุมากกว่า 65 ปี และมักมีปัญหาด้านการพูดที่คล่องแคล่วน้อยลง มีพฤติกรรมเปลี่ยนแปลง เช่น โมโหร้าย “โรคสมองเสื่อมจากภาวะสมองอุดตัน” มีเส้นเลือดอุดตันเล็ก ๆ ทำให้เกิดสมองเสื่อมตามมา “โรคสมองเสื่อมจากเชื้อวัวบ้า” เกิดจากทานอาหารที่ติดเชื้อไวรัสหรือไม่ทราบสาเหตุและโรคเอดส์ พาร์กินสัน ภาวะน้ำในโพรงสมองมากเกินไปหรือโรคสมองเสื่อมที่เกิดจากการสัมผัสกับสารพิษ เช่น เครื่องดื่มแอลกอฮอล์ สารเสพติดทั้งหลายหรือความผิดปกติของโรคทางกาย เช่น การเผาผลาญพลังงานในร่างกายผิดปกติ
อีกภาวะหนึ่งที่เราเจอได้บ่อยในบุคคลทั่วไปคือ อาการหลงลืมหรือความจำถดถอย ที่เกิดขึ้นถือเป็นรอยต่อของโรคก่อนกลายเป็นอัลไซเมอร์ โดยแบ่งได้เป็น 2 ประเภท คือ
1. อาการหลงลืมหรือความจำถดถอยเป็นผลจากอายุที่มากขึ้น(Age-Associated Memory Impairment : AAMI ) พบความชุก 40 เปอร์เซ็นต์ในคนที่อายุ 65 ปีขึ้นไป 2. อาการหลงลืมหรือความจำถดถอยที่ผิดปกติของเซลล์สมอง (Mild Cognitive Impairment : MCI)มีการลดลงของหน่วยความจำเมื่อเทียบกับเพื่อนอายุรุ่นเดียวกัน มีความจำของสมองคล้ายกับอัลไซเมอร์และประมาณ 15 เปอร์เซ็นต์ของคนที่มีภาวะสมองเสื่อม MCI จะพัฒนาเป็นโรคอัลไซเมอร์ทุกปี
การตรวจวินิจฉัยเบื้องต้นในคนไข้ที่เริ่มความจำไม่ดี เช่น อายุ 45 ปีขึ้นไปมักมีภาวะหลงลืมหรือความจำถดถอยลง ซึ่งคนไข้เหล่านี้จะเสี่ยงในการเป็นอัลไซ
เมอร์หรือไม่นั้น ปัจจุบันการตรวจทางการแพทย์ที่มีเทคโนโลยีที่ดีขึ้น โดยเราสามารถใช้เครื่องมือตรวจคนไข้กลุ่มนี้ได้ ดังนั้น การตรวจวินิจฉัยเบื้องต้นจึงสำคัญมาก เพราะการรักษาในช่วงต้นของอาการสมองเสื่อมจะช่วยชะลอการเกิดโรคสมองเสื่อมต่าง ๆ ตามมา ไม่ว่าจะเป็นการใช้ยา การแนะนำให้อ่านหนังสือ การเล่นเกม และการทำกิจกรรมส่งเสริมเรื่องความจำ โดยวิธีต่าง ๆ เหล่านี้จะทำให้สมองเสื่อมช้าลง
การแยกโรคก็ถือว่าสำคัญเช่นกัน โดยแพทย์ต้องวินิจฉัยได้ถูกต้องจึงจะสามารถรักษาได้ตรงจุด เพราะการรักษาโรคสมองเสื่อมแต่ละชนิดนั้นมีการบำบัดรักษาที่แตกต่างกัน เช่น โรคอัลไซเมอร์มีการใช้ยาบำบัดแต่ยังไม่สามารถทำให้หายขาดได้ แค่ช่วยชะลอตัวโรคเท่านั้น ซึ่งตัวยาจะไปเพิ่มสารเคมีตัวหนึ่งในสมองทำให้สมองเสื่อมช้าลง แต่โรคสมองเสื่อมบางอย่างไม่มีการขาดสารเหล่านี้ทำให้การรับประทานยาไม่เกิดประโยชน์ เช่น คนไข้ที่เป็นโรคสมองเสื่อมแบบ FTD จะมีอาการจิตเวชก่อน เช่น อาการซึมเศร้า จึงไปหาจิตแพทย์และรับการรักษาอยู่นาน จนกระทั่งพบว่าไม่ได้เป็นโรคจิตเวช แต่เป็นโรคสมองเสื่อม ซึ่งเราจะเห็นว่าแต่ละโรคจะมีการบริหารจัดการในการรักษาแตกต่างอย่างชัดเจน
ปัจจุบันเราจึงมีการใช้เครื่อง PET Scan ช่วยในการตรวจวินิจฉัยภาวะเริ่มต้นของสมองเสื่อมร่วมกับข้อมูลทางคลินิก จากการศึกษาพบว่าช่วยเพิ่มความแม่นยำในการวินิจฉัยได้สูงถึง 91.5 เปอร์ เซ็นต์เมื่อเทียบกับกลุ่มที่ไม่ได้ใช้แม่นยำเพียง 66 เปอร์เซ็นต์ และกรณีที่ผู้ป่วยเป็นโรคสมองเสื่อมแล้ว เครื่อง PET Scan จะช่วยในการแยกชนิดของโรคได้ว่าเป็นชนิดใด เพราะการรู้ชนิดของโรคจะทำให้ทราบว่าควรให้การรักษาอย่างไร หลักการของ PET Scan จะให้ข้อมูลเป็นภาพถ่ายที่ไม่เหมือนกับ MRI เพราะ MRI จะบอกแต่โครงสร้าง แต่ PET Scan จะบ่งบอกถึงการทำงานของเซลล์สมอง โดยสามารถวัดการทำงานของเซลล์สมองได้ด้วยการฉีดสารที่ไม่มีอันตรายต่อมนุษย์เข้าไปและสามารถดูการเปลี่ยน แปลงของเซลล์ในสมองได้ ซึ่งในส่วนที่มีความผิดปกติจะจับหรือใช้สารกลูโคสน้อยลง จากนั้นประมวลออกมาเป็นภาพการทำงานของเซลล์สมองในร่างกาย
ดังนั้นหากเรารู้จักสังเกตอาการภาวะสมองที่ถดถอยลง ได้แก่ ความจำไม่เหมือนเดิม มีพฤติกรรมหรืออารมณ์เปลี่ยน แปลงไป เช่น เคยเป็นคนสุภาพเรียบร้อยก็กลายเป็นคนโกรธง่าย หรือบางคนมีพฤติกรรมซ้ำ ๆ หรือบางคนก็เก็บตัว ไม่นอนกลางคืน หรืออาการที่พบบ่อยในระยะแรกเริ่ม ได้แก่ มีปัญหาด้านการเรียนรู้ ใครบอกอะไรแล้วไม่จำ ซึ่งแต่ก่อนไม่เป็น มีความลำบากในการใช้ภาษา นึกชื่อสิ่งของไม่ออกว่าชื่ออะไร มีปัญหาเรื่องความจำ จำเรื่องราวที่เกิดขึ้นในแต่ละวันไม่ค่อยได้ และอาการเหล่านี้เป็นมากขึ้นต่อเนื่องหรือบ่อยขึ้น ควรรีบมาตรวจเพื่อพบเจอรอยโรคก่อนนำไปสู่การรักษาที่ถูกต้องช่วยชะลออาการ
นอกจากการตรวจที่ทันท่วงทีแล้วการป้องกันถือว่าเป็นวิธีที่ดีมากที่เราต้องระวังปัจจัยเสี่ยง เช่น หลีกเลี่ยงอาหารที่มีไขมันสูง หวาน มัน เค็ม เครื่องดื่มที่มีแอลกอฮอล์ทุกชนิด พยายามออกกำลังกายให้สม่ำเสมอ หลีกเลี่ยงความเครียด ตรวจวัดความดันโลหิตสม่ำเสมอ ตรวจสุขภาพประจำปี ระวังเรื่องการใช้ยา ระวังอุบัติเหตุโดยเฉพาะที่ศีรษะ ทำกิจกรรมที่มีการฝึกสมอง เช่น อ่านหนังสือ การเล่นต่อคำ การเล่นไพ่ตอง การเล่นดนตรี การร้องเพลง เต้นรำ และสุดท้ายหากใครเริ่มมีอาการหลงลืมมากผิดปกติ หรือมีญาติผู้ใหญ่ที่ต้องสงสัยควรรีบพามาพบแพทย์เพื่อตรวจและรักษาได้ทันท่วงที.

เคล็ดลับสุขภาพดี : วิธีดูแลกระดูกสันหลังให้แข็งแรง…มีอายุยืนยาว
โรคกระดูก นับเป็นสาเหตุหนึ่งที่บั่นทอนสุขภาพ โดยเฉพาะในส่วนของกระดูกสันหลังเพราะมีความสำคัญต่อระบบโครงสร้างร่างกาย หากมีความผิดปกติจะทำให้เกิดการเจ็บปวด เช่น ปวดหลัง ปวดคอ สร้างความทุกข์ในการดำเนินชีวิตประจำวันได้

โดยวิธีดูแลรักษาทำได้โดย
1. ปรับเปลี่ยนพฤติกรรมการนอน ท่านอนที่ดีที่สุดคือ นอนหงาย เพราะเป็นการรองรับกระดูกสันหลังได้เป็นอย่างดี ไม่ควรนอนคว่ำ เพราะเราต้องบิดลำคอไปด้านใดด้านหนึ่งทำให้กระดูกสันหลังบริเวณคอเกิดความตึง อาจทำให้เกิดอาการล็อกหรือเรียกว่า คอตกหมอน หากเป็นเช่นนี้เวลานานจะทำให้ข้อกระดูกกดทับเส้นประสาทจะมีอาการปวดคอ ปวดศีรษะ และข้อต่อกระดูกเสื่อมเร็ว
2. การนั่งควรนั่งบนเก้าอี้ที่มีส่วนพนักพิงข้างหลัง หลังตรง ข้อศอกตั้งฉาก 90 องศา เท้าสัมผัสกับพื้น ลุกขึ้นและผ่อนคลายบ่อย ๆ
3. การยกของแต่ละครั้งควรพยายามให้หลังโค้งตามธรรมชาติอยู่เสมอ โดยวิธีการที่ดีและช่วยป้องกันไม่ให้ปวดหลังคือ การยกตะโพกและงอเข่าช่วยทุกครั้งเมื่อยกของหนัก
4. ควรรับประทานอาหารที่มีประโยชน์ต่อร่างกาย ไม่มีสารเคมี เช่น ผงชูรส ยาฆ่าแมลง ควรรับประทานอาหารที่มี Probiotics และ Prebiotics จะช่วยทำให้ร่างกายสามารถดูดซึมแร่ธาตุบางชนิดที่จำเป็นและมีความสำคัญต่อกระดูก แคลเซียม เหล็ก แมกนีเซียมและสังกะสี โดยอาหารที่มี Prebiotics พบมากในหัวหอม กล้วย กระเทียม หน่อไม้ฝรั่ง มะเขือเทศ ข้าวสาลี น้ำผึ้ง ส่วน Prebiotics ได้แก่ ผลิตภัณฑ์นมเปรี้ยว โยเกิร์ต
นอกจากนี้ควรสัมผัสกับแสงแดดในตอนเช้าประมาณ 20 นาทีต่อวันเพราะวิตามินดีจากแสงอาทิตย์จะช่วยเสริมสร้างกระดูก ควรดื่มน้ำสะอาดเป็นประจำทุกวันโดยเฉพาะช่วงเช้าเวลาตื่นนอนจะช่วยทำให้เซลล์ในร่างกายคงรูปทำงานได้ดีสามารถนำอาหารไปเลี้ยงกล้ามเนื้อต่าง ๆ ช่วยเสริมสร้างและซ่อมแซมส่วนที่สึกหรอของกล้ามเนื้อ กระดูก และช่วยหล่อไขข้อต่าง ๆ ของร่างกาย ที่สำคัญควรออกกำลังกายสม่ำเสมอ โดยบริหารกล้ามเนื้อและเส้นเอ็นก่อนเล่นเพื่อป้องกันการฉีกขาดของกล้ามเนื้อ.
